Personalized Plates

Standard-issue passenger plates (Bluegrass State and In God We Trust) and motorcycle plates, military plates, and special plates can be personalized.

The Kentucky Revised Statutes (KRS) 186.174 and Kentucky Administrative Regulations (KAR) 601 KAR 9:012 regulate personalized plates.

APPLICATION PROCESS
All personalized plate requests must be pre-approved by the Cabinet’s Personalized Plate Review Committee before applying with your County Clerk. To initiate the personalized plate application process, you must complete the following forms and take the necessary steps:
- Visit the Plate List Application page to check the availability of the character combination/s of your request.

You will do this by clicking on the image of the plate you are interested in and clicking on “PERSONALIZE THIS PLATE.”

- Once availability has been checked, submit your online request, and click on the “REQUEST THIS PLATE” button.

Specialty Plates
Any non-profit organization that is based in Kentucky or has a chapter located in Kentucky may apply for a special license plate under KRS 186.164.
APPLICATION PROCESS

Send an email request to [email protected], and they will send you a packet containing the documents that you need to apply.

Complete and return the documents listed below to [email protected] or mail them to:

LIST OF DOCUMENTS NEEDED:

A letter of request, which should include the mission or vision of the organization, a list of all chapters within Kentucky, and any other pertinent information about your organization that you would like to share with the Committee.

Form TC 96-333, Organization Application for a Special License Plate.

Proof of non-profit status from the IRS or Articles of Incorporation from the Kentucky Secretary of State.

Plate image designed per enclosed specifications. The packet includes an Adobe Illustrator file (*.ai) and an editable Adobe Document file (*.pdf). You may submit your design in either format.
Once the State receives your completed application, the Special License Plate Committee will review it at their next scheduled quarterly meeting.
